髓过氧化物酶(MPO)是一种四聚体含血红素蛋白,主要由中性粒细胞大量产生,在其中发挥重要的抗微生物作用。当粒细胞脱颗粒时,MPO会被释放到细胞外空间。作为中性粒细胞“呼吸爆发"的一部分,它能通过过氧化氢(H₂O₂)和氯离子(Cl⁻)生成次氯酸(HOCl)。此外,MPO还能利用过氧化氢氧化酪氨酸生成酪氨酰自由基。次氯酸和酪氨酰自由基均具有细胞毒性,可有效杀灭细菌及其他病原体。遗传性髓过氧化物酶缺乏症会增加个体患免疫缺陷的风险。
研究表明,MPO水平升高与冠状动脉疾病相关。2003年有研究提出,MPO可能作为胸痛患者心肌梗死的敏感预测指标。此后,大量文献(已发表逾百篇)进一步证实了MPO检测在心脏病患者中的临床价值。2010年的补充研究进一步优化了这一临床应用:通过联合检测MPO和C反应蛋白(CRP),相较于单独检测CRP,可更精准地预测死亡风险。
